How to Complete and Submit a Permit Application
 
After downloading, and printing the fire permit you need, fill out all of the information on the front of the form; use the itemized list to determine the cost of your permit, including the Permit Issuance fee.
 
Plan Review Submittal Requirement: One set of plans for the construction. For all other submittals, three sets of plans with all device related documentation ("cut sheets"), i.e., Fire Alarm, Fire Sprinkler, or Fire Extinguisher Systems, etc.
 
Method of Payment: At this time, checks or cash may be used for payment; credit cards and electronic transfer of funds may be available in the future.
 
When you have completed the form, and have calculated the fee for your project, bring the completed permit application, the plans for your project, and the appropriate fee to the Fire Prevention Division, located on the second floor at 333 Olympic Dr. Telephone: (310) 458-8915.
